INFORMATION WE MAY COLLECT

As part of providing our products and services to you, we may obtain “nonpublic personal information” and/or public information about you from the following sources:

  • Customer Information .  Customer information includes identifying information such as your name, address, telephone number, and demographic data about you.  It also includes information about your transactions with us such as claim history, premiums, and insurance coverage.
  • Financial Information .  Financial information is information covered by the Fair Credit Reporting Act and information such as your credit history, income, assets, and loan records.  It also includes information from consumer reporting agencies, public records, and data collection agencies.
  • Health Information .  Health information includes information about your health and medical history, and your insurance underwriting risk factors.

“Nonpublic personal information” is nonpublic information (including health information) about you that we obtain in connection with providing a financial product or other service to you.  For example, nonpublic personal information would include any personally identifiable financial information that is provided by you to a financial institution such as your account balance or payment history.

WHAT INFORMATION WE MAY DISCLOSE

It is our policy at Plateau not to disclose any non-public personal information about you to any non-affiliated third parties, except as permitted by law.  For example, we may disclose nonpublic personal information about you to help process and provide you with the financial product or other services which you have applied for and to government entities in response to subpoenas.  We may also disclose any information described above to companies that perform marketing services on our behalf or to other financial institutions with whom we jointly provide financial products or services.
